Assessment Purpose

The student will demonstrate the ability to assess, safely prioritise and plan care for a patient from the information given in the chosen scenario.

The assessment will include the identification and evaluation of socio‑economic and demographic factors that influence the patient’s usual functional patterns at home, as well as the alterations that have occurred since their admission to the acute inpatient environment.

Students will develop a plan of care from the assessment data, prioritising the nursing diagnoses, SMART Goals and, interventions, rationale and evaluation, demonstrating stage‑appropriate competence aligned with the NCNZ standards within the chosen scenario.

Rationale for planned nursing interventions and treatment will be demonstrated by clinical decision making, informed by evidenced based practice, principles of rehabilitation, Kawa whakaruruhau and is supported by integrated knowledge of pathophysiology and pharmacology.

Assessment Task/Activities

The student will choose ONE scenario out of FOUR given scenarios to complete the case study including socio economic, demographic factors and informed consent.

The student will use the Patient Plan of Care framework provided on Canvas to guide the case study which will include:

  • The holistic assessment data. This includes the changes to the patient’s three functional health patterns, review of three relevant body systems and physiological status following admission to the inpatient setting.
  • The three priority nursing diagnoses determined after critical analysis of the patient assessment information
  • Discussion of the Nursing Care Plan for the patient should include:
  • ONE of the three priority Nursing Diagnoses identified for the patient. Present in full NANDA format.
  • A patient goal for this nursing diagnosis – using the SMART format
  • Three nursing interventions planned
  • Two rationales for EACH intervention, demonstrating your depth understanding.
  • Rationale should be evidenced based where possible and reflect knowledge of pathophysiology and pharmacology where applicable.
  • Evaluation of care provided in relation to achievement of patient goal.
