MNSC842 Assessment Brief

Complete two (2) mini case reviews following the Waikato DHB medicine management workshop.

Each case review will briefly address the criteria below and provide a short reflection with objectives related to medicine management strategies.

Relates to learning outcomes 3, 4, 5:

  • Employ a critical approach to health assessment, diagnostic decision making and clinical reasoning.
  • Demonstrate knowledge in pharmacotherapeutics and its evidence base for safe medicine management.
  • Critically evaluate therapeutic plans and manage responses to drug therapy including adverse situations of drug misuse.

Criteria one 20%: Client’s presenting problem, health history and diagnostic data, relevant contextual data, including current drug therapies and goals of therapy.

  • Outline the problem, relevant history, diagnostic data (e.g. normal blood tests except for …).
  • Include the diagnosis and what has been ruled out. Be concise and reference.
  • Include all the current medications (use a table here) and goals of therapy (use references).

Criteria two 30%: Reasoning for the suitability of the preferred drug(s), includes: active substance and dosage, standard dosage schedule, standard duration of treatment appropriate for client and any drug interactions that may affect therapy outcomes.

  • Choose one or two medications that are relevant to the current presentation.
  • Explain why these medications are suitable. When will the therapeutic dose be achieved.
  • How do they work, what may inhibit their action.
  • Make sure this relates to the client.

Criteria three 40%: Specific information, instructions and warnings about the drug therapy are appropriate for client, includes effects and side effects of the drug, specific warnings, drug adherence and monitoring, assessment of understanding, challenges to adherence and proposed follow-up, any adjuvant non-pharmacological interventions that may benefit.

  • Discuss the side effects, e.g. what the medications do to the body, how does the body react.
  • Explain how will side effects these be managed / mitigated?
  • How do you know the client understands?
  • What are the barriers to / enablers of client concordance / response to instructions / warnings / follow-up?
  • What other non-pharmacological therapies may be included?
  • Briefly include the clients perspective / experience and / or use research literature.

Criteria four 10%: Critical reflection on case identifies one goal for future medicine management practice.

  • Briefly share what you have learnt about medication management.
  • For example … choose one: process of evaluating effects, individualising medication, precautions, patient education, nursing role, reasoning processes etc.
