Assignment Task
This assessment task requires you to refer to the case study of Tiana West provided.
The case study provides the first two steps of the CRC:
1. Consider Patient Situation; and
2. Collect Cues.
You are to address the subsequent five stages of the Clinical Reasoning Cycle:
1. Process Information
- Outline important abnormal assessment findings;
- Identify any relationships between the abnormal assessment findings;
- Briefly identify if nursing intervention is needed; and,
- Predict the potential outcome for the patient and the probable consequences if the nursing intervention were not to occur
2. Identify problem/issues
Based on your data in the Processing Information, discuss one main patient problem that will require an immediate nursing intervention by you as the Registered Nurse. Discuss why this is your priority
3. Establish goals
Using the SMART goal format, identify one desired goal relevant to the problem identified in the previous step of CRC.
4. Take action
Discuss one nursing intervention for the SMART goal in addition to the promotion of safety, healing and caring for the patient. Support the nursing intervention with evidence-based literature that is appropriate in the context of the case study.
5. Evaluate outcomes
Outline the process for evaluating the desired outcomes in this scenario.
