The proposed evidence-based project seeks to implement nurse-led care bundle protocols to reduce and prevent catheter-associated urinary tract infections (CAUTIs) in the inpatient nursing department of a large healthcare system. This essay evaluates the organizational culture and readiness to adopt the proposed change or intervention using the Organization’s Readiness to Implement Change (ORIC) tool. The paper proposes strategies that the health system can use to successfully implement and sustain change as well as improve care delivery.

Organizational Culture & Support for Change

At the core of an organization’s culture is how an entity turns its purpose into action and espouses shared values, beliefs, and behaviors that shape how employees interact among themselves, with patients, and other stakeholders. Aspects like leadership structure, mission and values, collaboration and team engagement, communication, and perception of the organization by employees shape the kind of culture that a healthcare organization creates or practices (Chalmers et al., 2025). The proposed EBP project will be implemented in a large healthcare facility that provides all services, from emergency to critical care, and nurses, as well as other providers, operate under increased pressure to deliver excellent patient care.

The facility’s leadership falls between a clan and hierarchical structure, implying that it combines the best aspects of the two approaches, making employees work collaboratively based on reporting structure, and our mission statement. The organizational culture is positive and committed to quality improvement, ensuring that the facility maintains its Magnet Status through high-quality care delivery (Santos, 2023). The mission and values of the organization focus on safety, compassionate care, innovation, and delivery of evidence-based practice interventions; aligning with the proposed intervention’s objectives.

Additionally, the hierarchical leadership model is based on shared governance, while the staff collaboratively work through interprofessional teams to deliver quality care. Effective communication in the organization ensures that all stakeholders can engage and interact, access and share information that influences their input. However, support and staff development, as well as implementation of continuous quality improvement initiatives and infrastructure, are critical (Melnyk et al., 2022). While the staff views the environment and readiness for change as positive, they want more support for quality improvement projects like the proposed EBP interventions. Organizational Readiness Assessment & Strategies to Improve Readiness

The readiness of an organization to implement change is essential when executing EBP interventions like nurse-led bundle of care protocols to reduce CAUTIs in healthcare facilities. Robertson et al. (2022) observe that assessment of an organizational readiness for change, like integrating patient safety practices through EBP interventions, leads to a better understanding of the perception of change and possible leadership support to attain change goals. As such, the Organization’s Readiness to Implement Change (ORIC) tool focuses on staff’s ability and competencies to initiate change, show consistency and persistence, and collaborate to execute the proposed interventions. The ORIC tool has 12 items programmed using a Likert scale, from 1 (disagree to 5 (agree), with the outcomes showing how staff feel they can implement change. The tool centers on change efficacy and commitment to change.

In this case, 12 nurses were surveyed in the nursing department, including critical care areas, and during both day and night shifts, using the ORIC tool. The results showed that 11 nurses strongly agreed on change with an average score of 4.9 out of 5. The results indicate a high level of readiness for change as staff expressed confidence in various areas of the assessment, like collaboration, leadership support, and resource availability and allocation.

Furthermore, the organization can implement additional strategies to facilitate the level of readiness for change. These include effective communication, more engagement and collaboration, support, and training for nurses who require more knowledge on the proposed EBP intervention (Santos, 2023). Additionally, leadership involvement, having a feedback loop, and cultural competency measures will be essential to addressing any possible barriers to the implementation, like change resistance and low collaborative levels, as well as resource allocation.

The assessment of the organizational culture demonstrates a high level, better support, and possible sustenance of an evidence-based practice change initiative. In their study, Clearly-Holdforth et al. (2022) observe the importance of assessing culture and readiness for EBP as it identifies strengths, challenges, and opportunities that may facilitate or hinder implementation of proposed change projects. The facility’s strengths include a strong nursing staff committed to change, a collaborative workplace environment, and a positive perception among employees. Possible challenges and barriers may entail potential resistance to change, allocation of sufficient resources, and time constraints due to the busy nature of the facility, particularly the critical care unit. However, stakeholder support, sufficient time for implementation, and adequate resource allocation will help attain a high organizational culture level to support the proposed EBP intervention.

Processes and Systems to Improve Quality, Safety, and Cost Effectiveness

Improving quality, safety, and cost-effectiveness for the facility to implement the proposed EBP change project requires effective processes and systems. In this case, the recommended processes and systems may entail leveraging healthcare technologies to improve workflow, data collection, and decision-making among providers like nurses and clinicians. These include clinical decision support systems (CDSS) and electronic health records (EHR) that empower providers to make better care decisions and improve hospital revenues by lowering operational costs and improving efficiency (Mun et al., 2024). Further, processes like active engagement of patients and their families, building a continuous learning system, and leveraging data and evidence-based learning can improve quality, safety, and reduce associated costs.

Stakeholders and Team Members for the Project

Stakeholder engagement is a core part of the EBP process, as the buy-in directly influences the success of change implementation. Stakeholders provide views, support implementation, and offer resources that are critical to the attainment of EBP project goals. Consequently, the main stakeholders in this case will include nurses and nurse managers, patients and their families, healthcare providers, organizational management, and regulatory agencies (Melnyk et al., 2022). Nurses and nurse managers will be the primary implementers of the proposed EBP and will gather all aspects of the project for better delivery. Patients and families will support the proposed intervention to reduce and prevent infections at all levels. Healthcare providers will collaboratively work with nurses to implement the project, provide feedback, and highlight areas for improvement. The organizational management will provide the required resources and support the project for the expected deliverables. The regulatory agencies will guide the implementation of the proposed EBP, ensuring that all actions and decisions adhere to existing regulatory requirements.

Information and Communication Technologies

Successful implementation of the proposed EBP project will require information and communication technologies like the EHR system, the CDSS, and databases such as PubMed and CINAHL to gather evidence and justify the proposed interventions. Further, the proposed EBP project will require emails and staff, as well as patient portals and dashboards to visualize the core results (Santos, 2023). These technologies will be incorporated into the setting through system upgrades by experts and internal users, especially super-users and the IT personnel, as well as nursing and clinical staff.

The integration of these technologies will help improve nursing practice and care delivery for both individuals and populations targeted by the intervention in various ways. Firstly, the technologies will enhance adherence to best practices like reduced use of catheters and other non-pharmacological approaches to lower infections among catheterized patients (Mun et al., 2024). Secondly, these technologies will increase nurses’ confidence and skills in catheter use and ways to leverage the care bundle to lower CAUTIs among patients. As such, the proposed technologies are core to the overall implementation of the proposed EBP project change for the targeted population and others.

Conclusion

A healthcare facility’s organizational culture offers a robust foundation to implement the proposed EBP project of lowering CAUTIs through nurse-led care bundles. Based on the evaluation, the facility illustrates strong readiness levels and support for change through the proposed EBP project to create the necessary change and lower the ever-rising cases of catheter-associated urinary tract infections (CAUTIs) among critical care patients. Through leadership support, effective planning, and integrated technologies, the project aims to enhance the quality of care and safety and improve care delivery within the facility for better patient outcomes.
