Provide a background on the clinical problem identified. Outline an action plan to implement the evidence-based project that includes: The recommended practice change. Proposed timeline for implementation. The tools or resources that will be needed to support the project. Identify the stakeholders, opportunities for innovation, and potential barriers to the practice change needed for project implementation. Who are the stakeholders who will be impacted?
