Categories and Example Causes:

1. People: Inadequate staff training, staff shortages.
2. Processes: Inefficient workflows, unclear medication orders.
3. Environment: Poor lighting in the medication preparation area.
4. Equipment: Outdated medication dispensing systems.
5. Materials: Lack of essential medications in stock.
