Please note, it is important to select a healthcare team member with a role that allows insight into the needs of the community, as this person will be integral in determining the priority health need of the community which is needed for the collaborative component of this assignment.

Component: Collaboration with Community Healthcare Team Member
Directions:
In the final portion of the community assessment project, you will create a presentation that details your analysis and plan for the community. Using the data identified through the windshield assessment, the data analysis, and the community interviews, work with the healthcare team member (the same person you interviewed) to determine the priority health need within the identified community and develop an appropriate plan of care. In collaboration with your community healthcare member develop a community diagnosis and an intervention plan that considers assessment findings with a focus on determinants of health, available resources, and the range of activities that contribute to health and the prevention of illness, injury, disability, or premature death.
