Your post must address the following:
Community/Population: Identify the specific community or population you have chosen and provide a brief rationale for your selection. Consider communities defined by geography, age, socioeconomic status, ethnicity, occupation, or other relevant characteristics.
Social Determinants of Health Identify at least two social determinants of health (SDOH) that significantly impact the health and well-being of your chosen population. Draw on course readings, peer-reviewed literature, or reputable public health sources (e.g., Healthy People 2030, CDC, WHO) to support your analysis.
Levels of Prevention For each SDOH identified, discuss how you would apply the primary, secondary, and/or tertiary levels of prevention to address the associated health challenges. Your response must include:
One specific example for each level of prevention as it relates to your identified SDOH issues
A clear explanation of why each prevention level is appropriate
Evidence-based rationale drawn from current research, public health guidelines, or established frameworks
